Housing Stabilization Services (HSS) is a Minnesota Medicaid benefit that helps seniors and people with disabilities find and keep stable, affordable housing. Our Housing Coordinators provide personalized support, and individuals can self-refer or be referred by a provider.

At Urban Living Solutions, our Housing Transition Services (HTS) help individuals move into stable, independent living environments tailored to their unique needs. We provide personalized support throughout every step of the process, making the transition smooth and successful.

Our services include:

• Developing individualized housing plans and budgets
   
• Assisting with applications, leasing, and required documentation
   
• Coordinating and supporting the entire moving process

• Training on tenant and property manager rights and responsibilities
   
• Assistance with applying for housing benefits and other resources
   
• Coordination of adaptive accommodations to meet individual needs
